The Ellison John Transitional Care Center is a 170 bed skilled nursing facility specializing in post-acute rehabilitation and skilled nursing services.
Our team of skilled healthcare professionals is committed to providing the highest level of care and customer service.
The Ellison John Transitional Care Center is recruiting experienced professionals and is currently accepting applications for a full-time Social Services Coordinator position.
Assists Director in the annual development, administration, and coordination of department policies and procedures.
Participates in discharge planning, and development and implementation of care plans and MDS.
Schedules the visits of outside consultants to the facility, including, but not limited to:
dentist, podiatrist, audiologist, psychologist, psychiatrist and optometrist.
Refers Residents/Families to appropriate social service agencies when the services are not provided on site.
Coordinates social service activities with other departments as necessary.
Reviews any resident/family concerns/grievances and forwards to the appropriate department manager and Administrator for follow up and resolution.
Interviews residents/family members as necessary to obtain information for completion of social history, assessments, progress notes, MDS and care plans while assuring all required federal and state documentation is completed accurately and timely.
Other duties as assigned.
